We respect the value of time.
By scheduling your appointment for therapy with Oak Tree Therapy Center and/or Andy Smith, LCSW, you agree to the terms of our Financial Services Policy below. This policy and its terms are formally acknowledged and accepted by each client prior to their first session as part of the New Client Information Form.
We hope you will respect that because we cannot practically rebook your session with less than 24 hours notice, we have to charge a Missed Appointment Fee for the session.
We respect your time in return. Your session will start within just a few minutes of the scheduled time.
Financial Services Policy
As with all medical appointments, you are responsible for your appointment-related fees. These might include things like insurance copays and deductibles required by your insurance provider, or session fees for our Self Pay clients.
In addition, please understand that appointment-related fees also include an automatic charge of $100 for sessions that are missed or cancelled within 24 hours. Sessions not attended within 15 minutes of the appointment time are considered missed.
Finally, you agree to pay a session rate of $100 should your insurance decline payment, if for instance, you’re no longer covered by the policy. (This is uncommon).
